ODL and US Club Soccer Forms

The list of forms below are being made available for teams to complete the registration process for participating in the ODL this Fall and next Spring.

The registration process requires the coach or team manager to complete an application for approval to play. The application requires basic contact details and information on the team requesting to participate. An e-mail address must be designated to receive confirmation of approval. Notification of approval or denial will be sent by September 1, 2011. If you do not receive notification, please send an e-mail to okdevleague@gmail.com ASAP with team name, age group and gender.

The following forms will also be required by ODL and US Club Soccer. US Club Soccer Medical releases for each player, copy of birth certificate for each player and coach/manager background forms.

ODL Forms:

#ODL1: TEAM REGISTRATION FORM		Last Updated: 12/12/07
This is the form which is required of all teams who are registering to play in the ODL. Please download the form from this site, complete it, and submit it to ODL.

US Club Soccer Forms:

#R002: PLAYER: REGIS. & MEDICAL TREATMENT AUTH.		Last Updated: 02/02/04
This is the form which is required of all players who are registering to play in the ODL with US Club Soccer. As an alternative to receiving this form from your club, players or parents may download the form from this site, complete it, and submit it to ODL.

#R004: COACH: RISK ASSESSMENT FORM		Last Updated: 08/10/10
This is the risk management form that each coach and administrator who will be working with players must complete. Send the original to ODL and keep a copy. This is the only paper necessary to register a staff member.
